All sections marked with a * are mandatory and must be filled in. This makes sure that we have all the information necessary to process your request as efficiently as possible.
Resource Details
Please provide us with as much information as possible about the subject that you would like resources for. Please use a separate form for each subject.
If your request is for a secondary school and the books are needed for a library collection, please tell us in this section of the form.
Pupil Information
Select relevant year group. More than one can be selected if required. For example, if the request is for Year One and Year Two, select both. There is also a Whole School option if needed. Please ensure you tell us how many children the request is for and the ability of the children.
Book Items and Audio Visual Items
You can tick as many of these as you require. By ticking yes to audio visual items, a menu will appear which will allow you to choose which items you would like. All items are subject to availability.
Teaching Period
Select either a full term or half a term for each project. Please note that due to popular demand, audio visual resources (objects, DVDs and talking books) are issued for half a term and can be renewed once if no other school is waiting for them.
Submitting Your Request
Once you have filled in the form, you will be able to submit your request. If it has been successfully submitted you will be given the option to fill out another request form or go back to the Education Library Service homepage. If after clicking submit nothing happens, please make sure you have completed all the mandatory sections and submit again.
